With over two decades of experience serving seniors and conducting over 30,000 hours of home visits, our physicians have developed a deep understanding of the challenges confronting our patients. Our practice is centered on caring for the needs of all our patients, including seniors, many of whom are often alone and vulnerable. We follow standard guideline directed medical therapy and avoid unnecessary testing/procedures.
Personalized Relationships
Our services are tailored to meet the unique needs of our clients. Clients consistently see the same team of medical professionals during each visit, eliminating the need to repeat medical history and investigations. Our doctors guide and assist patients and their families in selecting treatment options that consider the patient’s lifestyle, preferences, and living environment. We focus on understanding the person, not just the disease.
Clear Communication and Documentation
Each visit by our physician and ancillary service provider will be clearly documented with a PDF document generated within 24 hrs and shared with the patient and designated family members via web/email.
Phone Consultation and Availability
Doctors are available for phone consultation when asked for, during the physicians scheduled appointment hours. The doctor will no longer be difficult to reach.
Care coordination with Specialists
We don’t offer services in isolation. Depending on the conditions, patients may benefit from referrals to specialists. The lead doctor coordinates with other specialists when necessary. We have a vast network of reliable specialists. We understand the challenges of securing specialist appointments, and we strive to make this process seamless for our patients. After care is provided by the respective specialists, and we continue to monitor patients at home.
Ancillary Home Services
When necessary, the lead doctor will arrange and coordinate with ancillary home services, including physiotherapists, psychologists, and nutritionists.
Lab Testing
At home. When necessary, we can coordinate with lab services to provide simple blood and urine testing, X ray services at home avoiding a trip to the diagnostic centers or hospitals.
